Planning to be out of town? on vacation? You will need to complete a Prearranged Absence Form and turn it in prior to your child's scheduled absence. You can print a copy of the form below, pick a copy up at the front desk, or fill out the form electronically and email it to kellie_umphrey@lksd.org or joel_thomas@lksd.org.
